VT Providers Receive More Patient Info with CCD Project

Patient care summaries from Northeastern Vermont Regional Hospital (NVRH) are now available through the Vermont Health Information Exchange (VHIE) using VITLAccess, the secure provider portal. The addition of this important source of information allows providers like Caledonia Home Health Care & Hospice to see complete summaries of the care their patients received from the hospital.

NVRH is a community acute care and critical access hospital located in St. Johnsbury, VT. NVRH provides primary and preventative care, surgical and specialty care, inpatient and outpatient care and 24 hour, physician staffed emergency services. The care summaries available through VITLAccess are like the visit summaries given to patients when they leave the office and contain demographic information, a complete problem list, a current medication list, and recommended follow-up treatments.

VITLAccess is a secure portal to the VHIE that allows the exchange of medical information between providers. With a patient’s consent, any authorized provider involved in the treatment of that patient can use VITLAccess to view information about them from other healthcare organizations around the state. Currently, the VHIE receives patient information from all 14 Vermont hospitals, Dartmouth-Hitchcock Medical Center, all of Vermont FQHC’s, 163 primary and specialty care locations, five visiting nurse associations, and three commercial labs.

In addition to NVRH, the VHIE continues to receive care summaries from Grace Cottage Hospital, Mt. Ascutney Hospital and the UVM Medical Center. Click here for more information about VITLAccess.
