Primary Care Connectivity
There are a number of reasons to connect your primary care medical practice to the Vermont HIE. Connection to the Vermont HIE allows for:
- Transmittal of clinical data to the Blueprint for Health central clinical registry (DocSite).
- Transmittal of any required data if you are a participating provider under an accountable care organization.
- Transmittal of Immunization Data to the Vermont Department of Health Immunization Registry.
- The receipt of laboratory or test results directly into your electronic health record (EHR) sytem from a participating hospital, commercial lab, or pharmacy.
- Transmittal of lab or test orders directly from your EHR to a participating hospital, commercial lab, or pharmacy.
Primary Care Interface Types:
Patient Demographics – Admit, Discharge and Transfer (ADT)
Demographic data are used to uniquely identify a patient, and may include data elements such as name, date of birth, and address. Demographic data is sent from the health care organization to VITL in an Admit, Discharge or Transfer (ADT) message. Patient demographic data are used to facilitate the aggregation of a shared community health record, and may also be forwarded to other registries, such as the Blueprint central clinical registry, at the request of the sending organization. Hospitals are senders of ADT messages.
Laboratory orders are the instructions provided by a physician to a lab to conduct a particular type of test. Laboratory orders can now be sent electronically from a health care organization’s electronic health record (EHR) system and routed through the Vermont HIE to laboratories. This electronic transmission is much faster and accurate than sending the order by phone, fax, or in paper form.
Results from laboratory tests, such as blood tests or urinalysis, are important for the diagnosis and treatment of health condidtions. Laboratory results can be sent electronically from laboratories and routed through the Vermonht HIE to one or more receiving health care organization’s electronic health record (EHR) systems. This creates a timelier lab result and provides discrete data elements that can be incorporated into a patient’s electronic health record.
Transcribed Reports (radiology, pathology and other departments)
Transcribed reports are physician reports from specialty departments, such as radiology or pathology, that describe the results from certain procedures or tests. Rather than sending these reports by fax, phone, or paper, they can now be sent electronically from the specialty department, through the Vermont HIE, to a providers electronic health record (EHR) system.
Clinical Summary - Continuity of Care Document (CCD)
A clinical summary generally contains recent patient data such as a problem list, medications, allergies, immunizations, lab results, patient notes, and other summarized data. Clinical summaries are transmitted as a Continuity of Care Document (CCD). A clinical summaries is used to convey patient information at a specific point in time, and is not a complete medical record. CCD's are sent from a health care organization to the Vermont HIE and are can be routed to registries, such as the Blueprint central clinical registry, allowing a Blueprint community health teams to assist with management of a patient population.
Immunization data is sent from a practice or medical organization’s EHR and routed through VITL to the Vermont Department of Health Immunization Registry.
