Data Analysis & Quality Improvement
VITL will help with assuring the quality of new data being entered into an electronic health record (EHR) or practice management (PM) systems, in remediating old data in systems, or in preparing data for transmission outside of a system.
In implementing various EHR and PM systems, the client services team has a great deal of experience in what is necessary to create and maintain structured data that will allow for ease of recording and reporting. We are knowledgeable in the data requirements for initiatives such as:
- Meaningful Use (MU)
- Patient-Centered Medical Home (PCMH)
- National Committee For Quality Assurance (NCQA)
- Uniform Data System (UDS)
- Accountable Care Organizations (ACO)
and how to "cross-walk" these for maximum system efficiency. We also recognize the importance of practice/organization-specific quality improvement (QI) and patient panel management, and can work with you to get data that is responsive to your needs.
Fixing or remediating existing data can be a challenge that few practices have the time or staff to accomplish. However, remediation often becomes necessary when changing systems, as a part of preparing to send data to other sources, or simply when a substantial error or inconsistency in reporting is discovered. VITL can assist by helping to evaluate the focus and scope of a data remediation project, talking with system vendors if necessary, finding technical resources, and providing data analyses.
The comparison of data becomes important when "cleaning up" records in a system to assure accuracy, or in finding errors in data that is kept in two or more systems.The client services team can perform comparisons that will help uncover discrepancies and errors, allowing progress with remediation or further system compatibility work.
