Information collection 

The VITL website may collect personal information you supply to us voluntarily. Web forms on vitl.net may request your personal information to help VITL staff contact you or process your request. These include but are not limited to the “Contact Us” form, the “Subscribe” form, and the form for opting-out of data sharing through the Vermont Health Information Exchange.  The personal identifiable information that we may obtain from you may include your first and last name, email address, phone number, and address.  

IP addresses 

When you visit our website, we may collect information about  how the website is accessed. This information may include your computer or mobile device’s internet protocol (IP address), the pages that you visit, the time and date of your visit.  An IP address is a number assigned to your computer or internet-enabled device whenever you access the internet. It allows computers and servers to recognize and communicate with one another. IP addresses from which visitors appear to originate may be recorded for IT security and system diagnostic purposes. We use third party usage analytics tools to monitor and analyze the use of the website through Google Analytics. Google Analytics does not reveal your name or identifying information to VITL. The information is only collected to improve our website. To provide website visitors with more choice on how their data is collected by Google Analytics, Google has developed the Google Analytics Opt-out Browser Add-on: https://tools.google.com/dlpage/gaoptout/.  The add-on communicates with the Google Analytics JavaScript (ga.js) to indicate information about the website visit should not be sent to Google Analytics.
